Job summary

University Hospital Southampton NHS Foundation Trust is seeking a Discharge Officer to support safe, timely hospital discharges and improve patient flow.

You will manage a caseload, coordinate with wards, nurses, social care, and community teams, and communicate clearly with patients and families. The role is based at Southampton General Hospital with NHS induction and ongoing training.

Qualifications

• Proven ability to manage deadlines and multitask.
• Experience in an administrative or patient-facing role in healthcare is desirable.
• Good literacy and numeracy as part of GSCE standard or equivalent.

Responsibilities

• Coordinate safe discharge from admission, ensuring plans are realistic and person-centred.
• Collaborate with nurses, doctors, therapists, social workers and community partners to deliver on-time discharge plans.
• Monitor patient discharge information using hospital digital systems and escalate delays as needed.
• Liaise with care agencies and social services to arrange assessments and placements.
• Act as advocate for patients and families, ensuring discharge information is clear and voices are heard.
