Job summary

HCRG Care Group is recruiting for a Band 4 Discharge and Flow Senior Coordinator based at Salisbury District Hospital, Bath. You will lead a team to ensure safe, timely discharges and support patient journeys with MDT collaboration.

You will line‑manage Discharge Coordinators across BSW Community Hospitals, coordinate admin actions, and maintain records to support capacity planning. The role requires a proactive, communicative leader with experience in discharge planning.

Qualifications

• NVQ Level 3 in Care, Business Administration, or equivalent experience.
• Confident IT skills, including Microsoft Office (Word, Excel), Email, and MS Teams.
• Experience working in a health or social care environment.
• Experience working within a multi‑disciplinary team.
• Ability to prioritise workload and stay organised in a fast‑paced setting.
• Previous supervisory experience.
• Proactive, motivated, and able to identify and support improvements.

Responsibilities

• Line-manage Discharge Coordinators across BSW Community Hospitals.
• Coordinate all administrative actions that support timely, safe discharge.
• Ensure patients and families understand the discharge pathway and timeframes.
• Visit patients on wards to provide regular updates on discharge progress.
• Maintain strong communication with MDT colleagues and partners.
• Monitor discharge readiness and referral processes within the system.
