Job summary

West London Mental Health Trust is seeking a motivated Discharge Coordinator to join the Hospital Discharge Team. You will manage a caseload of patients with varying complexity and coordinate safe, timely discharges from admission through to discharge.

Working closely with wards, community services, social care and external partners, you will remove barriers to discharge, lead on complex discharge planning, and promote best practice to improve patient outcomes and experience.

Responsibilities

• Coordinate safe, timely discharges from admission through to discharge
• Lead on complex discharge planning and ensure early bed release guidelines are followed
• Engage with ward staff, community services, social care and external partners to remove barriers to discharge
• Attend meetings (Daily Reason to Reside, MDT, professional and family meetings) to support patient flow and decision making
• Promote nurse-led discharges at weekends and develop staff competency within the discharge team
