Job summary

Qualifications

• NVQ Level 3/Level 3 Diploma in Health & Social Care or equivalent relevant experience.
• Care Certificate or willingness to complete within first 6 months of commencement in post.
• GCSE equivalent in English and Mathematics.

Responsibilities

• Arrange safe discharges for ward patients and liaise with multidisciplinary team to determine care and services needed.
• Discuss with patients and relatives to establish support needed for discharge.
• Coordinate with social services, discharge management team and district nurses.
