Job summary

Mersey and West Lancashire Teaching Hospitals NHS Trust inKnowsley/St Helens & Whiston Hospitals is seeking a TCU/Discharge Co-ordinator to join the Gynaecology ward team. You will support discharge planning, liaise with MDT, patients and carers, and ensure the patient journey ends with a safe, timely discharge.

The role requires excellent communication, a caring approach, Enhanced DBS clearance for children and adults, and NVQ Level 3 or a Diploma in Healthcare.

Qualifications

• NVQ Level 3 plus training and experience in a Healthcare setting.
• Diploma level in a Healthcare setting or have the equivalent knowledge gained through training and experience.
• Enhanced DBS clearance for children and adults.

Responsibilities

• Liaise with wards to keep the discharge list up to date for the following morning.
• Communicate effectively with patients, carers and MDT members to support discharge.
• Promote clear communication with ward staff and other departments regarding the role.
