Job summary

Wirral University Teaching Hospital NHS Foundation Trust is seeking an experienced Discharge Coordinator to manage complex discharge planning within the Transfer of Care Hub. You will collaborate with the MDT to assess needs, coordinate care and arrange safe, timely discharges for patients with varied care requirements.

The role requires RGN with degree-level education, active NMC registration, and ongoing professional development.

Qualifications

• Registered nurse with degree-level education and active NMC registration.
• Evidence of ongoing professional development (CPD).
• Experience in coordinating hospital discharges and creating individual care plans.
• Willingness to undertake mentorship/preceptorship training or equivalent.

Responsibilities

• Lead assessment and discharge planning for patients with complex needs.
• Collaborate with MDT to ensure safe, timely discharges.
• Assist in arranging suitable post-discharge care facilities and services.
• Engage with patients and families to support care decisions.
