Job summary

County Durham & Darlington NHS Foundation Trust is seeking a Discharge Co-ordinator for our gastroenterology ward to ensure safe, timely discharges and smooth transitions for patients, involving collaboration with social workers, district nurses, and the multidisciplinary team.

The role requires clear communication, careful documentation, and ability to manage competing priorities in a busy hospital setting; closing date 07 October 2026, with pre-employment checks.

Qualifications

• Relevant NVQ Level 3/Level 3 Diploma in Health & Social Care or equivalent relevant experience.
• Care Certificate or willingness to complete within first 6 months of commencement in post.
• GCSE equivalent in English and Mathematics.
• Short courses relevant to undertake patient and clinical care duties.

Responsibilities

• Discussion with patients and relatives to establish what support is needed to facilitate discharge out of hospital.
• Discussion with multidisciplinary team members including social services, the discharge management team and district nurses.
• Liaising with the discharge lounge to arrange discharge home.
