Qualifications

• Coordinating with multiple stakeholders to meet outcomes.
• Providing advice or signposting to services.
• Collecting data and monitoring information to assess impact.

Responsabilités

• Identify a cohort of patients and support their personalised care.
• Coordinate patient healthcare with the patient and clinician.
• Work with GPs and primary care professionals to manage a caseload.
• Support patients and carers before and after clinical conversations.
• Assist patients with managing their needs and making appointments.
• Help eligible individuals access personal health budgets where appropriate.
