The Methodist Church in Britain? not used here. Taurus Healthcare Limited is seeking a non-clinical co-ordinator within the North & West PCN Care Home Team to streamline care planning, MDT coordination, and information sharing across care homes and practices.

You will help ensure smooth discharge processes and maintain electronic records. The role involves liaising with primary, secondary and social care partners, supporting the vaccination programme, and leading communications such as quarterly

Qualifications

• GCSE English and Maths at minimum level is required.
• Proficient IT and digital communication skills are essential.

Responsabilités

• Act as the central point of contact for the North & West PCN Care Home Team.
• Coordinate Personalised Care and Support Plans and Structured Medication Reviews for residents.
• Coordinate Palliative Care and Care Home MDT meetings with practice leads.
• Support care home ward rounds by liaising with homes and ensuring patient lists are updated.
• Facilitate discharge and handover between care settings across the health and social care system.
• Record and circulate MDT actions and maintain electronic patient records.
• Support rollout of seasonal vaccination programmes with practices and care homes.
• Build relationships across primary, secondary, community, mental health, social services and ambulance services.
• Organise and attend PCN meetings, including quarterly cross-PCN peer meetings.
• Lead with the team on a quarterly newsletter to care homes.
• Elevate safeguarding risks with the PCN safeguarding team and practices.
