OneMedical Group is seeking a Care Navigator to ensure patients receive an efficient, supportive experience when accessing our services. You will handle patient requests, record data accurately and direct individuals along suitable clinical or administrative pathways.

Full role-specific training will be provided, including clinical escalation procedures, safeguarding, and information governance to support high-quality patient care.

Qualifications

• Excellent communication and organisational skills.
• Ability to stay accurate and professional in a busy environment.
• Confident using digital systems and providing patient-focused support.

Responsabilités

• Receive, review and process patient requests and ensure information is recorded accurately.
• Direct requests to the appropriate clinical or administrative pathway.
• Signpost patients to primary care, community, external or self-referral services.
