Job summary

Practice 3 Keir Hardie Health Park is seeking a Care Navigator to be the initial point of contact for patients, welcoming them in person or remotely and directing them to the appropriate healthcare resource. You will coordinate appointments, manage simple signposting, and support the multidisciplinary team while ensuring data protection and safeguarding standards are upheld.

Responsibilities

• First Point of Contact: Welcome patients and visitors in person, via telephone, or through digital/online consultation platforms with a professional, calm, and welcoming demeanor.
• Active Signposting & Triage Support: Conduct non-clinical assessments of patient needs to direct them to the right resource (GPs, practice nurses, mental health practitioners, social prescribers, or local community services).
• Appointment & Workflow Management: Book, amend, and cancel appointments, handle prescription requests, and manage day-to-day administrative tasks using clinical systems.
• Communication & Collaboration: Act as a central communication link between patients, the multidisciplinary clinical team, hospitals, and external health agencies.
• Confidentiality & Compliance: Maintain accurate, confidential patient records in strict compliance with NHS data protection, GDPR, and safeguarding guidelines.
