Job summary

Northamptonshire Children’s Trust is seeking a Personal Advisor to support care leavers aged 16–25. You will manage Pathway Plans addressing education, housing, health and relationships, and provide personalised support to separated young people while considering immigration status.

The role involves duty rotas, occasional evenings, and county-wide travel. You will work closely with the Practice Manager and partner agencies to improve outcomes and participate in care councils and events,

Qualifications

• Strong understanding of legislation relating to looked-after children and care leavers.
• Ability to build meaningful relationships with young people, including those who are harder to engage.
• Flexibility to work evenings or travel outside the county.

Responsibilities

• Be allocated individual young people and help assess and develop their Pathway Plans covering education, housing, health and relationships.
• Provide personalised support to all care leavers, including separated young people and the impact of their immigration status.
• Work with the Practice Manager, Team Manager, colleagues and partner agencies to improve service delivery and outcomes.
• Represent the Leaving Care Service at participation events and Care Council meetings.
• Contribute to a programme of skills-based activities for young people.
• Work within a duty rota to ensure consistent support across the service.
