Rutland County Council is seeking a committed Personal Advisor to Care Leavers to join our Leaving Care Service. You will work with care leavers aged 16-25 to support their transition to adulthood, helping them achieve stability and independence.

You will collaborate with social workers, housing providers, education and training providers, health professionals and partner agencies to develop pathway plans, provide independent living skills training and secure housing and education opportunities,

Qualificações

• NVQ Level 3 or equivalent qualification.
• Experience of working and engaging positively with young people (age 16-25).
• Knowledge and experience of working with Care Leavers and supporting them to access education, employment and training opportunities.
• Understanding of Pathway Planning and legislation relating to Children Looked After and Care Leavers.
• Experience to support the development of young people's independence, including budgeting, tenancy sustainment and life skills.
• Willingness to travel across the county with a full driving licence.

Responsabilidades

• Support allocated young people and maintain case records and statutory recordings.
• Contribute to assessments and Pathway Plans with active involvement from young people.
• Help access education, employment and training opportunities.
• Provide practical housing, budgeting and independent living guidance.
• Coordinate with partner agencies to ensure holistic support.
• Help develop confidence, resilience and independence in young people.
• Assist in implementing assessments, interventions and pathway plans.
