Job summary

Peterborough City Council is seeking an experienced Personal Advisor to support leaving care young people as they transition to adulthood and independent living.

You will manage a caseload, conduct assessments, develop Pathway Plans and collaborate with partner agencies to improve outcomes for care leavers. The role requires safeguarding knowledge and a flexible approach to meet needs, including occasional unsocial hours.

Qualifications

• The ideal candidate will have experience of working with care leavers, vulnerable young people or within Children’s Social Care.
• Strong assessment, communication and partnership-working skills, together with an understanding of safeguarding.

Responsibilities

• Manage a caseload of individual care leavers in line with relevant legislation, policies and procedures.
• Assess the needs of young people and provide appropriate support and intervention to help them achieve positive outcomes.
• Develop, plan and review Pathway Plans with care leavers, ensuring young people remain at the centre of all planning and decision-making.
• Support young people with accommodation, education, employment, training, finances, health and independent living.
• Assess financial needs and support care leavers with budgeting for accommodation and maintenance.
• Undertake regular visits to young people in their homes and other appropriate settings.
• Maintain accurate and confidential case records and ensure all documentation is completed in line with departmental standards.
• Attend and contribute to reviews, planning meetings, case conferences and other professional meetings.
• Work collaboratively with Children’s Social Care, education, health, housing and other partner agencies.
• Identify and respond appropriately to safeguarding concerns involving vulnerable young people.
• Provide services in accordance with the Children Act and Leaving Care legislation, guidance and regulations.
• Promote equality, diversity and social justice throughout all aspects of practice.
• Work flexibly to meet the needs of young people, including lone working and occasional unsocial hours.
• Participate in supervision, appraisals, practice observations and ongoing professional development.
• Maintain up-to-date knowledge of relevant legislation, policies, procedures and best practice.
