Job summary

Leeds City Council’s Care Leaver Service is seeking three Personal Advisors to support young people transitioning from care. You will build trust, advocate for rights, and collaborate with housing, health and youth agencies to ensure comprehensive pathways to independent adulthood.

Responsibilities include assessing needs, developing pathway plans, and providing practical guidance while working within a dynamic team. A DBS check and flexible, sometimes out-of-hours work are required.

Qualifications

• NVQ Level 3/4 in Care or willingness to start within 3 months and complete within 12 months.
• Ability to form constructive relationships with diverse young people.
• Willingness to work flexible hours and travel as required.

Responsibilities

• Assess needs and develop pathway plans for care leavers.
• Advocate for young people with partner agencies and professionals.
• Provide guidance and support to help young people transition to independence.
• Maintain accurate case records and participate in safeguarding procedures.
• Work collaboratively with teams to ensure high-quality service delivery.
