Job summary

LiveWest is advertising a part-time Support Worker role within the New Horizons Scheme covering Launceston, Liskeard and Bodmin in East Cornwall. You will provide person-centred support to customers with complex needs to help them gain independence, confidence and stability in their communities.

You will work outside standard hours, complete risk assessments, maintain records, and contribute to service performance.

Qualifications

• Experience of supporting people with a range of needs.
• Ability to build effective, professional relationships.
• Confidence working independently, including lone working.
• Ability to make informed decisions and take accountability for actions.
• Experience maintaining accurate records and action plans.
• Willingness to work flexibly to meet service and customer needs.
• Understanding of working with vulnerable people.
• Safeguarding training or willingness to complete.

Responsibilities

• Deliver high-quality, person-centred support tailored to individual needs.
• Building positive and professional relationships with customers, colleagues and partner agencies.
• Supporting customers to identify goals and develop personal support plans.
• Promoting independence, inclusion and participation to reduce social isolation.
• Completing risk assessments and managing risk in line with policies and procedures.
• Maintaining accurate records and contributing to service performance targets.
• Working safely, including lone working, while maintaining clear professional boundaries.
