Job summary

Together Housing Group is seeking a part-time Support Officer to provide person-centred support to older people and vulnerable adults. This 25-hour-per-week role is based at Potters Place, Chesterfield (S40 2FA) and offers rewarding opportunities to help customers maintain independence, develop skills and access the right services.

Qualifications

• Experience in a customer-focused support environment.
• Experience supporting older people or vulnerable adults.
• Excellent communication and interpersonal skills.
• Caring, compassionate and person-centred approach to supporting others.
• Understanding safeguarding and protecting vulnerable people.
• Ability to manage challenging situations calmly and empathetically.
• Strong organisational skills and ability to manage a varied workload.
• Ability to work independently while seeking advice when needed.
• Experience collaborating with partner organisations and external agencies.

Responsibilities

• Deliver person-centred support tailored to each customer’s needs, goals and aspirations.
• Support older people, vulnerable adults and customers experiencing homelessness to maintain their independence and improve wellbeing.
• Complete and regularly review support plans, risk assessments and outcome-focused support with customers.
• Provide practical and emotional support, including crisis response when required.
• Coordinate with health professionals, social care teams, housing services and partner agencies to coordinate support.
• Help customers explore housing options and prepare for moving into independent or more appropriate accommodation.
• Encourage customers to develop life skills, confidence and resilience for long-term outcomes.
• Promote safeguarding, wellbeing and safety of customers.
• Facilitate activities, resident meetings and community engagement that reduce isolation.
• Maintain accurate records, support plans, case notes and monitoring information using internal systems.
• Respond professionally to customer enquiries, concerns and complaints.
