Job summary

Tŷ Hafan is seeking a Family Support Worker to join the Family Support Team on a 6 month fixed term contract in the hospice and community setting. The role is part-time, 15 hours per week, across Monday to Wednesday, 10:00am to 3:00pm, and may involve occasional weekend work to support activities.

You will facilitate groups and provide one-to-one emotional and practical support to families, with a Diploma in Childcare or Health and Social Care, and the ability to travel across the catchment

Qualifications

• Diploma required and travel across catchment area.
• Experience facilitating groups, events or activities.
• Experience providing emotional or practical support to families.

Responsibilities

• Organise and facilitate events and groups in the hospice and community.
• Support equitable access for families across the catchment area.
• Contribute to engagement strategies using social media and other communications.
• Provide one-to-one emotional support and mentoring as directed.
• Use assessment and risk awareness to identify when families need more support.
• Maintain accurate records and contribute to reporting.
• Collaborate with colleagues and volunteers across the organisation.
