Job summary

Action for Children is seeking a Family Support Practitioner in Gwent on a fixed-term full-time contract. The role focuses on delivering targeted early family interventions, one-to-one and family support, and collaborative work with schools, social services and health partners.

You will manage a caseload, participate in TAF meetings, and deliver time-limited interventions with families. Cardiff/ Gwent-based, the post emphasizes safeguarding, multidisciplinary teamwork, and delivering

Qualifications

  • Strong work experience of working with children, young people and their families providing one to one and family work in the community.
  • Experience of delivering targeted early interventions for children and young people.
  • Proven record of working collaboratively with schools, social services and health partners and responding to safeguarding concerns.
  • Experience in managing and holding a caseload of 10 minimum including managing risk and safeguarding.
  • Experience of attending and inputting to Team Around the Family (TAF) meetings to meet the needs of vulnerable children and families.
  • Experience in delivering time-limited interventions including one to one support and group work delivery and facilitation
  • An ability to work on your own initiative but as part of a wider team.

Responsibilities

  • Working with a range of professionals to ensure the children and young people you support develop their full potential.
  • Working with a Practice Team Leader, Clinical Psychologist to explore the needs of children and young people where emerging social, emotional, behavioural and emerging mental health issues have been identified.
  • Delivering a time limited intervention up to 12 weeks in collaboration with the child or young person and their family.
  • Liaising closely with your Practice Team Leader, colleagues and our partners to ensure compliance an excellence.
