Job summary

Cornwall Partnership NHS Foundation Trust is seeking a part-time Mental Health Assessment Professional to work across the Mid Cornwall area, travelling to community locations to assess and signpost patients. You will collaborate with GPs and social care services to ensure seamless onward care and support.

Qualifications

• Registered mental health professional
• Evidence of post basic training
• Current professional registration
• Sign Off Mentor (SOM) qualification

Responsibilities

• Community-based role with travel across Cornwall
• Manage referrals to secondary mental health services via triage and initial assessment
• Collaborate with GPs, referring agencies and community mental health teams
• Support development during period of transformation and implement new ways of working
• Provide holistic mental health assessments and signpost next steps across NHS, social care and VCSE services
