Job summary

Cornwall Partnership NHS Foundation Trust is recruiting a Mental Health Practitioner (Band 6) for the West Home Treatment Team in Redruth. The role is full-time or part-time on a permanent basis, delivering community-based, crisis-focused mental health care to adults of working age.

Join a MDT with a strong focus on home treatment and reducing hospital admissions, while supporting flexible working patterns where possible within Cornwall.

Qualifications

• Registered professional registration with NMC, HCPC or SW.
• Experience delivering mental health crisis care in community or home settings.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Provide urgent clinical assessment in crisis situations within the Home Treatment Team.
• Deliver home-based interventions to support patients to remain at home safely.
• Participate in MDT planning and coordinate care with partners and services.
• Engage with patients and carers to promote recovery and engagement in treatment.
