Job summary

Your Life Your Way is seeking a part-time Care Manager to oversee local authority funded care packages and write care, risk and person-centred plans. The role covers supervising Personal Assistants and coordinating with the Scheduling Department, clients, families and health professionals.

Requirements include two years’ health and social care experience, a valid DBS or willingness to obtain one, a full driving licence with vehicle access, and a Level 5 qualification or willingness to undertake

Qualifications

• Excellent written and verbal communication, planning, organisational and analytical skills.
• Able to work independently and manage competing priorities.

Responsibilities

• Manage local authority funded care packages in an assigned area.
• Complete care assessments, risk assessments and person-centred plans within required timescales.
• Support the Registered Care Manager with day-to-day care responsibilities.
• Line manage, supervise and support Personal Assistants, including observations and return-to-work discussions.
• Support onboarding and development of Personal Assistants to build the right team for each client.
• Formulate initial rotas before passing them to the Scheduling Department.
• Work with clients, families, representatives, social workers and health professionals.
• Promote safeguarding, dignity, privacy, independence and choice.
• Capture feedback and complaints appropriately.
• Monitor care delivery, safeguarding, whistleblowing, budgets, KPIs and regulatory requirements.
• Use internal IT systems accurately.
• Take part in the 24-hour on-call rota for clients and staff outside office hours.
