Job summary

Together Housing Group is seeking a part-time Support Officer to provide person-centred assistance to customers, including older people and those experiencing homelessness. The role covers coordinating support plans, safeguarding and wellbeing, and working with health and housing partners to improve outcomes.

Based at Potters Place in Chesterfield (S40 2FA), you’ll engage with residents, assist with housing options, and help maintain safe, welcoming schemes while keeping accurate records.

Qualifications

• Experience in a customer-focused support environment.
• Experience supporting older people, vulnerable adults or disadvantaged individuals.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver person-centred support tailored to each customer’s needs, goals and aspirations.
• Support older people, vulnerable adults and customers experiencing homelessness to maintain independence and wellbeing.
• Complete and regularly review support plans, risk assessments and outcome-focused support with customers.
• Provide practical and emotional support, including responding to crisis situations where required.
• Coordinate with health professionals, social care teams and housing services.
• Help customers explore housing options and prepare for moving into independent accommodation.
• Promote safeguarding and wellbeing and interact with partner organisations to promote safety.
• Facilitate activities and community engagement to reduce isolation.
• Maintain records using internal systems and respond to customer enquiries and complaints.
