Job summary

HCRG Care Group is seeking a Band 5 Physiotherapist to join the Reablement Team based in Bath and surrounding areas, with a base at Calne Family Health Centre. The role focuses on promoting independence through rehabilitation and equipment provision for adults at home or in care settings.

You will assess patients, plan rehabilitation, and work within an integrated multi-professional team to maximise outcomes while maintaining high professional standards.

Qualifications

• Registered as a Physiotherapist with the Health and Care Professionals Council.
• Current, valid full driving licence and suitably insured.
• Broad range of clinical experience through student placements.
• Evidence of continuing professional development.
• Experience applying research-based information to practice.
• Competence in a wide range of physiotherapy skills.
• Able to work flexibly and sensitively.
• A valid UK driving licence and vehicle for business use.
• Knowledge of legislation affecting professional practice.

Responsibilities

• Manage own caseload, evaluate interventions and document to CSP standards.
• Act as a role model, ensuring high quality, professional standards.
• Work within an integrated multi-professional team.
• Assess, interpret, plan and implement physiotherapy treatment within guidelines, with support.
• Undertake assessments, care planning and evaluation.
• Help avoid hospital admission and support independence.
