NHS Jobs is seeking a Band 5 Physiotherapist to join the Reablement Team based in Bath and surrounding areas. The role is part-time (30 hours per week) at Calne Family Health Centre, supporting adults at home or in care settings to promote independence through rehabilitation and equipment provision.

You will work with a multi-professional team, manage your own caseload, and contribute to care plans with a focus on reducing inpatient stays and enabling patient participation in decisions.

Qualifikationen

• Essential: MCSP Dip PT / BSc / BSc (Hons) PT; Registered with HCPC.
• Current, valid full driving licence and suitably insured.
• Broad range of clinical experience through student placements.
• Evidence of continuing professional development.
• Competence in a wide range of physiotherapy skills.

Aufgaben

• Manage own caseload with evaluation of interventions and documentation.
• Act as a role model, maintaining high professional standards.
• Work within an integrated multi-professional team.
• Assess, plan and implement physiotherapy treatment per professional guidelines.
• Undertake assessments, care planning and evaluation.
• Aim to avoid hospital admission and maximise independence.
