Job summary

Berkshire Healthcare NHS Foundation Trust is seeking a community physiotherapist for a five-month bank post in Wokingham. You will assess patients in the community and develop discharge plans within a multidisciplinary team, covering falls and reablement pathways on weekday day shifts totalling 37.75 hours.

Requirements include HCPC registration, experience with older people, and a driving licence for community-based work.

Qualifications

• HCPC registration required.
• Experience with older people.
• Driving licence and access to a car for community work.

Responsibilities

• Assess patients in the community, providing a high standard of initial assessment.
• Liaise with MDT members on treatment and management care plans, working closely with therapists.
• Create and evaluate care plans, including discharge planning and onward community referrals.
• Devise physiotherapy treatment plans and goals with patients, relatives, carers and the wider team.
• Use and report on standardised and non-standardised assessments to analyse highly complex needs in relation to daily living.
