Job summary

HCRG Care Group in Surrey Heath and Farnham seeks a Band 3 Community Rehabilitation Assistant to join our Intermediate Care Team. You will work in patients' homes, supporting rehabilitation and independence, in collaboration with Physiotherapists, Occupational Therapists, Nurses, and other healthcare professionals to provide high-quality, person-centred care.

Qualifications

• Passionate about supporting people to regain independence.
• Caring, compassionate, and respectful approach.
• Excellent communication and interpersonal skills.
• Can work independently while also being an effective team member.
• Adaptable, reliable, and motivated to deliver high-quality care.
• Previous experience in a health or social care setting (desirable but not essential).

Responsibilities

• Visit patients in their own homes across Surrey Heath and Farnham, supporting independence and rehabilitation goals.
• Deliver person-centred care and rehabilitation plans developed by the therapy team.
• Support activities of daily living and medication support where appropriate.
• Assist with exercise programmes and rehabilitation activities prescribed by Therapists.
• Empower individuals to maintain independence, dignity and wellbeing.
• Monitor and document patient progress using a tablet in the community.
• Report changes in patient wellbeing and work with the multidisciplinary team to deliver safe care.
• Undertake administrative duties to support patient care and service operations.
• Maintain knowledge and skills to respond to changing needs of patients and the healthcare system.
