Job summary

Sue Ryder Duchess of Kent Hospice is seeking an experienced Clinical Nurse Specialist to provide high-quality, person-centred palliative care across Berkshire West. You will deliver direct assessment, advocacy, and support to patients and carers facing life-limiting illness, while collaborating with the wider healthcare team.

Based at the Theale base with travel to the In-Patient Unit and Palliative Care Hub, you will work day and night shifts and pursue Non-Medical Prescribing development as

Qualifications

• NMC registered with Non Medical Prescribing qualification or willing to complete
• Clinical Case Load Management and Community experience
• Ability to support adjustments for diverse patient needs
• Skilled in accessible communication with patients and families
• Commitment to inclusion and patient advocacy

Responsibilities

• Provide direct assessment, support and advocacy to service users and carers dealing with complex, distressing and life-limiting illnesses
• Respond to telephone calls, offer appropriate clinical advice, and support the wider team to deliver high-quality care across settings
• Based at Berkshire West Community office in Theale with travel to Duchess of Kent Hospice In-Patient Unit and South Oxfordshire Palliative Care Hub in Wallingford
• Travel will be coordinated from Theale base
• Support ongoing development, including Non-Medical Prescribing course as part of professional growth
