Job summary

Change Grow Live is seeking an Assertive Outreach Recovery Coordinator at HMP Bristol to assess and engage service users, ensuring they access the right intervention at the right time.

You will work with Clinical Substance Misuse teams, hold a caseload for 1:1 keyworking, run groups, and help coordinate Release Planning with partners to support smoother transitions.

This full-time, permanent role (37.5 hours/week) offers a supportive team, pension, wellness benefits, and learning opportunities.

Qualifications

• Strong understanding of substance misuse issues and experience in a similar field.
• Excellent verbal and written communication skills and IT proficiency.
• Proactive, able to work on own initiative with good time management and prioritisation.
• Good working knowledge of mental health interventions and best practices.
• Able to work in partnership with a wide range of professionals and stakeholders.
• Ability to manage change while prioritising service user needs.

Responsibilities

• Work with Clinical Substance Misuse team to develop and implement care plans for those receiving clinical prescribing.
• Complete initial assessment and welfare checks as part of Duty Shifts.
• Hold a caseload of clients for 1:1 keyworking.
• Facilitation of groups.
• Coordinate with Connecting Communities Teams, Healthcare, Prison partners and External Support Agencies for Release Planning.
