Job summary

Change Grow Live is seeking a Sessional Recovery Coordinator at HMP Bristol to engage with service users, assess needs and access the right interventions. You will work with clinical teams to develop care plans, facilitate groups and coordinate release planning with partner organisations.

The role requires experience in substance misuse, strong communication, and the ability to work collaboratively in a complex, secure environment. DBS checked role and visa not sponsorship.

Qualifications

• Strong understanding of substance misuse issues and related care pathways.
• Experience facilitating groups and managing group dynamics in challenging settings.
• Excellent verbal and written communication with solid IT skills.
• Proactive, able to work independently with effective time management.
• Good working knowledge of mental health interventions and services.
• Collaborative team player able to work with a wide range of professionals and agencies.
• Able to manage change prioritising the needs of service users.
• Understand information governance and safeguarding requirements.
• Open to learning and developing skills to support colleagues and service users.

Responsibilities

• Develop and implement care plans for those on clinical prescribing with attendance at reviews.
• Complete secondary screening assessment for prisoners as part of the integrated health model.
• Facilitate group sessions and activities for service users.
• Coordinate release planning with Connecting Community Teams, Healthcare, prison partners and external agencies.
