Ryde House Group is seeking a Care Co-ordinator to lead our Outreach Services, overseeing day-to-day delivery and ensuring quality, person-centred care for service users.

The ideal candidate has a QCF Level 3 in Health and Social Care or equivalent experience as a Person in Charge, Senior, or Manager. A driving licence is essential for this role, with duties including on-call coverage in a small team.

• QCF Level 3 in Health and Social Care or equivalent experience as a Person in Charge, Senior, or Manager.
• A driving licence is essential with use of company vehicles, as required.
• On-call duties shared within a small team.

Responsibilities

• Manage day-to-day delivery of Outreach Services.
• Complete medication and health & safety audits and ensure compliance.
• Conduct initial assessments and onboarding of new service users.
• Interview and assess candidates for new staff members.
