Waythrough is seeking a dedicated Recovery Coordinator to support individuals experiencing prescription and OTC dependency. You will deliver person-centred assessments, create recovery plans and provide regular keyworking sessions, both face-to-face and remotely.

You’ll collaborate with GPs, pharmacies and mental health teams to ensure safe, coordinated care and harm reduction, enabling healthier, more supported lives.

Formación

• Experience in substance misuse, mental health, healthcare or related support services.
• Ability to manage a caseload and deliver person-centred support.
• Transparent safeguarding and risk management understanding.
• High standards of record keeping and professionalism.
• Strong communication with clients and multi-disciplinary teams.

Responsabilidades

• Manage a caseload of individuals experiencing prescription or OTC dependency.
• Conduct comprehensive assessments and develop personalised recovery/treatment plans.
• Deliver structured one-to-one keyworking sessions, in person and remotely.
• Monitor progress, risks and outcomes, adjusting support as needed.
• Coordinate care with GPs, pharmacies, pain management and mental health teams.
• Attend MDT meetings to ensure coordinated care and service improvement.
