Job summary

Change Grow Live in Worksop is recruiting a Recovery Coordinator to assess and engage service users, coordinating personalised recovery journeys and ensuring access to the right interventions. The role focuses on opiate caseloads within a multidisciplinary team and requires proactive, compassionate care.

You will coordinate assessments, support planning and group participation, while managing risks and upholding high standards of reporting and collaboration with internal and external teams.

Qualifications

• Experience in managing caseloads.
• Commitment to multi-agency working and establishing partnerships.
• Accomplished written and verbal communication and IT competency.
• Ability to update and maintain records accurately and meet deadlines for reports.
• Excellent time management and ability to work on own initiative.
• Strong team working and interpersonal skills to support colleagues.
• Good knowledge of drug-related harm and risks.
• Good knowledge of mental health interventions and practices.
• Understanding of drug and alcohol issues and related experience.

Responsibilities

• Support service users from entry into the service and through their recovery journey.
• Provide screening, assessment, recovery planning and onward referral.
• Reduce drug and alcohol related harm for service users and the wider community.
• Promote carer, service user and community involvement.
• Advocate for access to partnership services.
• Work with service users to support social reintegration and recovery.
• Design and prepare materials for groups/pods and deliver structured content.
• Engage in debriefing and evaluation with co-facilitators and review progress.
