Job summary

Somerset Council is recruiting a Social Worker for the Hospital Interface Service at Yeovil District Hospital. The post offers a 37-hour week in a fast-paced hospital environment with opportunities to develop practice onsite and participate in a rotational Saturday rota.

Starting salary £37,280 rising to £41,777, and 30 days’ annual leave. This hospital-based role requires a recognised Social Work qualification, registration with Social Work England, and a vehicle.

Qualifications

• Hold a recognised Social Work qualification and current registration with Social Work England.
• Knowledge of Social Care practice, legislation and issues affecting adults and carers.
• Access to/use of a vehicle is an essential requirement of the role.
• Experience working in fast-paced, hospital or multidisciplinary settings is desirable.
• Demonstrates resilience, empathy and ability to manage risk and competing priorities.

Responsibilities

• Undertake ongoing assessments of allocated cases, including risk, independence and need.
• Manage a caseload of varying complexity and respond to referrals from multiple sources.
• Work within relevant legislation, policy and procedures, including reviews and risk assessments.
• Engage with clients to assess needs and agree levels of support.
• Support safe discharge planning and admission avoidance through collaboration with health professionals.
