Job summary

Somerset Council is seeking a Hospital Social Worker for an onsite, full-time role within a fast-paced hospital interface service. You will assess risk, support independence and coordinate discharge planning with health colleagues.

The role requires a recognised social work qualification, registration with Social Work England and access to a vehicle. Rotational Saturday work and 30 days annual leave are included as part of the package.

Qualifications

• Recognised Social Work qualification and current Social Work England registration.
• Knowledge of Social Care practice, relevant legislation and issues affecting adults and carers.
• Access to a vehicle is essential for the role.
• Experience in a fast-paced, multidisciplinary hospital environment.
• Ability to balance competing priorities and make defensible professional decisions.
• Commitment to helping people maintain independence and achieve positive outcomes.

Responsibilities

• Undertake ongoing assessments of allocated cases, including risk and independence assessments, to protect service users.
• Manage a caseload of varying complexity with referrals from multiple sources.
• Work within relevant legislation, policy and procedures, including statutory reviews and risk assessments.
• Assess client needs in consultation with them and other professionals, agreeing appropriate levels of support.
• Engage sensitively with people who may be refusing a service where there may be risk to themselves or others.
• Support safe and timely discharge planning and admission avoidance in collaboration with health teams.
