Job description

We are seeking a dedicated and compassionate Social Worker to join our Hospital Team in Warwickshire on a temporary basis at a competitive rate of £31.97 per hour. This role is vital in providing high-quality social work support to patients and their families, ensuring they receive the care and assistance they need during their hospital stay. As a member of our multidisciplinary team, you will play a crucial role in assessing social needs, developing care plans, and facilitating discharge processes.

Your key responsibilities will include:

  • Conducting comprehensive assessments of patients' social, emotional, and practical needs.
  • Collaborating with healthcare professionals to create and implement effective care plans.
  • Providing emotional support and guidance to patients and their families during challenging times.

To be successful in this role, you will need the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, CQSW, or equivalent).
  • Registration with Social Work England.
  • Proven experience in a hospital or healthcare setting, with a strong understanding of relevant legislation and policies.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and families.
  • Strong organizational and time management skills, with the ability to work effectively under pressure.
