Job summary

Care Dorset Limited is seeking an Occupational Therapist to support people in community-based services, enabling independence and improving quality of life. The role focuses on outcome-focused assessments, tailored therapeutic plans, and a person-centred, strengths-based service aligned with the Care Act 2014.

You will deliver OT assessments, set SMART goals, and work with a multidisciplinary team to ensure safe, effective, and goal-oriented outcomes in reablement settings.

Qualifications

• Qualified Occupational Therapist eligible to work in the UK.
• Experience delivering person-centred OT assessments and interventions.
• Ability to plan, implement and review reablement-focused goals.

Responsibilities

• Deliver timely, person-centred OT assessments and interventions for adults with disabilities in bedded reablement settings.
• Set, monitor, and review SMART goals in partnership with individuals to regain independence.
• Manage a diverse caseload with complex cases and collaborate with MDT for integrated care.
• Provide high-quality OT services in line with legislation, local policies and standards.
• Support and mentor the team; guide Reablement Assistants in planning and reviews.
• Promote use of equipment and adaptations to maximise independence.
• Maintain accurate records using the Nourish system and communicate with carers and partners.
• Engage in CPD and MDT meetings to contribute to service improvement.
