Job summary

JustOT is seeking a Part-time Community Reablement Occupational Therapist to support individuals in accessing a Reablement service. Applicants must hold current HCPC registration and have experience in health or social care, ideally in Reablement or community settings.

A full driving licence and access to a vehicle are essential. You will undertake OT assessments, help maximise independence, provide equipment/adaptation recommendations, and work with the therapy team to deliver person-centred,

Responsibilities

• Undertake Occupational Therapy assessments for individuals currently receiving bridging support and awaiting commencement of Reablement.
• Identify opportunities to maximise independence, reduce ongoing care needs and support timely progression into Reablement services.
• Provide recommendations regarding equipment, adaptations, moving and handling, and rehabilitation interventions to support safe and effective care.
• Work collaboratively with the wider therapy team, Reablement staff and partner agencies to deliver person-centred, outcome-focused support.
• Maintain accurate records and contribute to care planning, risk assessment and review processes in line with professional and organisational standards.]
