Job summary

Care Dorset Ltd is seeking an Occupational Therapist to support adults in community-based services, delivering outcome-focused assessments and tailored therapy plans to improve independence and wellbeing.

You will work with a multi-disciplinary team, promote a reablement approach, and contribute to ongoing development of a person-centred service, while documenting care in Nourish and coordinating with carers and partners.

Qualifications

Responsibilities

• 1. Assessment and Intervention: Deliver timely, person-centred Occupational Therapy assessments and interventions to adults with physical disabilities and age-related conditions within bedded reablement settings.
• 2. Case Management: Manage a diverse caseload with complex cases, ensuring safe, effective, goal-oriented outcomes.
• 3. Service Delivery: Provide high-quality Occupational Therapy aligned with legislation and professional standards.
• 4. Team Support and Development: Coach and mentor the team; guide Reablement Assistants in intervention planning.
• 5. Consultation and Advocacy: Promote equipment use and adaptations to maximise independence.
• 6. Communication and Documentation: Maintain accurate records using Nourish; liaise with families and partners.
• 7. Professional Development: Engage in CPD and MDT meetings.
• 8. Operational Contribution: Attend weekly caseload reviews and represent Care Dorset in external forums.
