Job summary

Guardian Jobs is seeking a qualified Occupational Therapist with HCPC registration for a home-visit role within a Reablement Service. You will carry out holistic assessments, develop tailored intervention plans and review progress to support independence.

You'll work with a multidisciplinary team, contribute to short-term, strengths-based services, and participate in a seven-day operation with weekend rotation. The role offers chances for supervision and professional development.

Qualifications

• Registered Occupational Therapist with HCPC.
• Experience in assessment, planning and reviewing case work.
• Ability to work independently and with others and escalate cases as needed.

Responsibilities

• Take the lead on assessing, planning, and reviewing complex cases to deliver high-quality outcomes.
• Deliver person-centred functional assessments and interventions to promote independence.
• Collaborate with a multidisciplinary team including OT, carers, sensory specialists and support staff.
• Provide expert guidance and support to colleagues, underpinned by social care legislation and clinical best practice.
• Contribute to a seven-day service with weekend rotation and some bank holidays.
