Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced Occupational Therapist to join our Integrated Discharge Team. You will assess functional needs, plan discharge pathways, and coordinate support to promote independence and safe transitions from hospital to home or suitable care settings.

You will work with a multidisciplinary team including nurses, physiotherapists, social workers, and community partners.

Qualifications

• Assess functional needs and determine discharge requirements.
• Coordinate safe, timely discharge pathways.
• Educate and supervise junior staff on OT role and practice.
• Participate in planning and evaluating the discharge service.

Responsibilities

• Assess patients to determine discharge needs and suitable pathways.
• Plan and coordinate roles across a multidisciplinary team.
• Lead information management to ensure high-quality data and reporting.
• Support service development within the acute trust and integrated care.
