Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an experienced occupational therapist to work on Alton Ward at Clay Cross Hospital, supporting discharge planning and rehabilitation within a community hospital setting.

You will carry a clinical caseload, provide supervision to junior staff, and collaborate with nursing, physiotherapy and social care colleagues to coordinate safe, timely discharges and patient-centred care across the locality.

Qualifications

• Provides specialist occupational therapy assessment and intervention for patients in a community hospital ward.
• Manages clinical risk and develops person-centred treatment plans.
• Collaborates with nursing, physiotherapy, medical teams and Adult Social Care to support discharge planning.

Responsibilities

• Lead and supervise junior staff and students.
• Contribute to service development, audit and quality improvement.
• Coordinate timely and safe discharges with the MDT and external agencies.
• Advocate for patient-centred outcomes and accurate documentation.
