Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced Occupational Therapist to join the Integrated Discharge Team on a Band 6 position. The role encompasses discharge planning, mentoring junior staff, and coordinating multidisciplinary care to ensure safe, timely transitions from hospital.

The post requires HCPC registration and a focus on delivering patient-centred care across 7 days per week. Interviewed candidates will attend in early November 2026.

Qualifications

• Diploma/degree in OT with HCPC registration
• Postgraduate diploma or training relevant to physical conditions
• Evidence of continuing professional development

Responsibilities

• Assess, plan and coordinate safe, timely discharges via appropriate discharge pathway
• Manage a caseload of patients with varying diagnoses and disabilities
• Educate and train the team, supervise and conduct annual reviews
• Participate in planning, development and evaluation of discharge services
• Lead information management to ensure high-quality data collection and reporting
