Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking an experienced Occupational Therapist to join the Integrated Discharge Team. You will assess functional needs, plan discharge pathways, and coordinate support with a multidisciplinary group to ensure safe and timely patient transitions.

Qualifications

• Qualified Occupational Therapist with UK registration
• Experience in hospital discharge planning and MDT collaboration
• Ability to supervise junior staff and deliver annual reviews

Responsibilities

• Assess, plan and coordinate safe, timely discharges via appropriate pathways.
• Manage a caseload with diverse diagnoses and complex needs.
• Educate and train the team on OT roles and supervision.
• Participate in development and evaluation of discharge services.
• Lead information management to ensure high quality data reporting.
