FCMS (NW) Ltd seeks a District Nurse/Community Nurse to join our clinical team in a mobile clinic setting, delivering care to patients with cardiovascular disease (CVD) and COPD in the community.
The role involves holistic assessments, initiating and amending medications as appropriate, health promotion and lifestyle guidance, and maintaining accurate records in EMIS. NMC registration must be current; part-time, bank contract with flexible hours is offered.
Opportunity for a Nurse to join our Clinical team!
The post holder will provide proactive reviews and support for patients with cardiovascular disease (CVD) and/or chronic obstructive pulmonary disease (COPD), working in line with clinical competence and local pathways. They will undertake holistic assessments, initiate and amend prescribed medications where appropriate, identify unmet needs and escalating health concerns, and deliver health promotion and prevention interventions, including lifestyle advice, weight management support, wellbeing guidance, opportunistic screening and vaccination advice.
Working within a mobile clinical unit, the post holder will deliver community-based care, providing assessment, advice, reassurance and ongoing support, while signposting or referring patients to appropriate services when required. They will promote self-management, maintain accurate clinical records on EMIS, uphold confidentiality and data protection standards, maintain current NMC registration, work autonomously within their scope of practice, and participate in supervision, appraisal and continuing professional development.
