Nurse – Non Medical Prescriber | Blackpool

We are seeking a dedicated and compassionate Nurse to join our team!

• Salary – £24.25 to £27.98 an hour Depending on experience

Main duties of the role

The post holder will provide proactive reviews and support for patients with cardiovascular disease (CVD) and/or chronic obstructive pulmonary disease (COPD), undertaking holistic assessments to identify physical, mental and social needs. They will initiate and amend prescribed medications as required, deliver health promotion and prevention interventions, including lifestyle advice, weight management support, wellbeing guidance, opportunistic screening and vaccination advice, and identify any unmet needs or escalating health concerns to ensure appropriate action is taken.

Working within a mobile clinical unit, the post holder will deliver community-based consultations, providing assessment, advice, reassurance and ongoing support. They will promote self-management, signpost or refer patients to appropriate services when required, accurately record all activity on EMIS using appropriate clinical coding, and maintain confidentiality and data protection standards. The role requires autonomous practice within scope of competence, adherence to the NMC Code of Conduct, and active participation in supervision, appraisal and continuing professional development.
