Job summary

Fcms Nw is seeking a nurse to join our clinical team on a bank contract in Blackpool. The role focuses on delivering community-based care to patients with cardiovascular disease and COPD, within a mobile clinical unit.

You will perform holistic assessments, adjust prescribed medications where appropriate, and promote health through lifestyle advice, screening, and vaccination guidance, while maintaining accurate EMIS records and professional standards.

Opportunity for a Nurse to join our Clinical team!

• Contract – Bank
• Location – Newfield House, Vicarage Lane, Blackpool, Lancashire, FY4 4EW
• Salary – £19.78 to £23.83 an hour Depending on experience

Main duties of the role

The post holder will provide proactive reviews and support for patients with cardiovascular disease (CVD) and/or chronic obstructive pulmonary disease (COPD), working in line with clinical competence and local pathways. They will undertake holistic assessments, initiate and amend prescribed medications where appropriate, identify unmet needs and escalating health concerns, and deliver health promotion and prevention interventions, including lifestyle advice, weight management support, wellbeing guidance, opportunistic screening and vaccination advice.

Working within a mobile clinical unit, the post holder will deliver community-based care, providing assessment, advice, reassurance and ongoing support, while signposting or referring patients to appropriate services when required. They will promote self-management, maintain accurate clinical records on EMIS, uphold confidentiality and data protection standards, maintain current NMC registration, work autonomously within their scope of practice, and participate in supervision, appraisal and continuing professional development.
