Job summary

FCMS (NW) Ltd seeks a dedicated nurse to join the Blackpool team on a part-time bank contract. The role offers flexible hours and site presence at Newfield House, Vicarage Lane.

You will deliver community-based cardiovascular and COPD care, conduct holistic assessments, initiate medications, and provide health promotion, vaccination advice, and self-management support.

EMIS documentation, patient confidentiality, and adherence to professional standards are essential.

Responsibilities

• Provide proactive reviews and support for patients with cardiovascular disease and/or COPD, undertaking holistic assessments.
• Initiate and amend prescribed medications as required.
• Deliver health promotion, prevention interventions, including lifestyle advice, weight management support, wellbeing guidance, screening and vaccination advice.
• Record activity on EMIS and maintain confidentiality and data protection standards.
• Work autonomously within scope of competence, adhere to the NMC Code of Conduct, and participate in supervision and CPD.

• Salary – £24.25 to £27.98 an hour Depending on experience

Main duties of the role

The post holder will provide proactive reviews and support for patients with cardiovascular disease (CVD) and/or chronic obstructive pulmonary disease (COPD), undertaking holistic assessments to identify physical, mental and social needs. They will initiate and amend prescribed medications as required, deliver health promotion and prevention interventions, including lifestyle advice, weight management support, wellbeing guidance, opportunistic screening and vaccination advice, and identify any unmet needs or escalating health concerns to ensure appropriate action is taken.

Working within a mobile clinical unit, the post holder will deliver community-based consultations, providing assessment, advice, reassurance and ongoing support. They will promote self-management, signpost or refer patients to appropriate services when required, accurately record all activity on EMIS using appropriate clinical coding, and maintain confidentiality and data protection standards. The role requires autonomous practice within scope of competence, adherence to the NMC Code of Conduct, and active participation in supervision, appraisal and continuing professional development.
