Job summary

Fcms Nw is seeking a nurse to join our clinical team on a bank contract in Blackpool. The role focuses on delivering community-based care to patients with cardiovascular disease and COPD, within a mobile clinical unit.

You will perform holistic assessments, adjust prescribed medications where appropriate, and promote health through lifestyle advice, screening, and vaccination guidance, while maintaining accurate EMIS records and professional standards.

Qualifications

• Provide proactive reviews for patients with CVD/COPD and implement care per local pathways.
• Undertake holistic assessments and manage prescribed medications within scope.
• Maintain EMIS records, ensure confidentiality and data protection.
