Reporting to the All Age Continuing Care Clinical Lead Nurses, you will be part of a dedicated team working within the health and social care system.

Reporting to the All Age Continuing Care Clinical Lead Nurses, you will be part of a dedicated team working within the health and social care system.

Main duties of the job

Committed to high quality patient and family experiences, you will be working collaboratively alongside other partners and stakeholders to ensure individuals needs are appropriately met. You will be responsible for the specialist Continuing Healthcare assessments, reviews, and case management of the health needs of individuals either in their own homes, residential, specialist accommodation or nursing accommodation and act as a point of contact, referral, and advice, completing regular reviews for health funded individuals and ensuring compliance with NHS National Framework Guidance. Case Management may also include safe commissioning of safe services for individuals, supporting with Court of Protection & Deprivation of Liberty processes, engagement in safeguarding, appeals, professional disputes and complaints, support to find appropriate placements etc.
